In October 2013, the International Scientific Association for Probiotics and Prebiotics (ISAPP) organized a meeting of clinical and scientific experts on probiotics (with specialties in gastroenterology, pediatrics, family medicine, gut microbiota, microbiology of probiotic bacteria, microbial genetics, immunology, and food science) to reexamine the concept of probiotics. They define probiotics as "live microorganisms that, when administered in adequate amounts, confer a health benefit on the host." They also differentiated between products containing probiotics and those containing live or active cultures and established the following criteria:
Because there's a huge variety of probiotic products available on the market, many people struggle to find the right one for them. "There are many different strains of bacteria in different combinations within different probiotic supplements," explains Brooke Scheller, a clinical and functional nutritionist. "For example, a probiotic may contain a single strain of bacteria or many. It may also contain other vitamins, minerals, or other ingredients that may tout health benefits," she says. There are many different dosages, delivery systems (powder, tablets, capsules), and formulations (refrigerated vs. shelf-stable), and some probiotics also contain prebiotics, which basically act as fertilizer for the probiotics. Given that you can get probiotics from the food you eat, you don't necessarily need to take a supplement, and Keatley says she prefers food sources of probiotics to supplements. However, she adds that "There are times when you really need a boost." One example might be during or after a course of antibiotics, as long as you have your doctor's OK, since antibiotics kill both good and bad bacteria: "Seeding your gut with good bacteria can lay the groundwork for a faster recovery and less constipation and diarrhea," Keatley points out. In seven people, however, these bacterial levels persisted for more than five months after the treatment ended. “We never expected they would survive more than a few weeks,” Walter says. A follow-up analysis determined that these seven people had begun the experiment with lower levels of B. longum in the first place. Afif Ghannoum said the key insight, one that he called groundbreaking, is that his father’s team found that fungi and microbes were cooperating in the construction of the plaques. He said election microscope images show the filaments of the fungi being incorporated into the carbohydrate ‘armor’ that the bacteria start to secrete once enough individual cells have gathered at a certain location. Without the addition of the digestive enzyme to their product, he said that the plaques, once adhered to the intestinal wall, would be difficult to attack.
Digestive problems. The best evidence for probiotics is for reducing diarrhea, especially following antibiotic use. A 2010 review from the Cochrane Collaboration concluded that probiotics shorten episodes of acute infectious diarrhea. And in 2011, a Health Canada monograph stated that products containing certain probiotics (such as Lactobacillus rhamnosus GG) help manage acute infectious diarrhea and antibiotic-associated diarrhea. A 2012 research review in the Journal of the American Medical Association (JAMA) found that probiotics reduced the risk of antibiotic-associated diarrhea by 42 percent—but many of the studies had flaws, so these findings should be interpreted with caution. A 2013 Cochrane review of 23 trials also concluded that probiotics may be effective for preventing antibiotic-related diarrhea. However, two large, well-designed studies, in the Lancet in 2013 and the Annals of Internal Medicine in 2014, found that probiotics were no better than a placebo in preventing diarrhea in older people taking antibiotics. A review of 19 studies, published in Gastroenterology in 2017, found that probiotics reduce the risk of Clostridium difficile-related diarrhea in hospital patients, especially when the supplements were started during the first two days of antibiotic treatment.
Prebiotics are fuel for the good bacteria (probiotics) in your gut; without prebiotics, probiotics can’t do their job. There are tons of prebiotics in whole fruits and vegetables, including onions, garlic, bananas, asparagus, and artichokes. The price is great on these, but definitely BEWARE! I do not recommend taking 2 pills in the beginning if you have not taken probiotics in the past! I had been eating very clean for a couple months, but got a sinus infection and was recommended by a nutritionist to take a probiotic to boost gut health since I was on antibiotics. The second Elinav study, “Post-Antibiotic Gut Mucosal Microbiome Reconstitution Is Impaired by Probiotics and Improved by Autologous FMT” involved a mouse model and human patients. One cohort of mice and a parallel human group was given the same probiotic strain used in the first study to see if they had more success repopulating their gut microbiota after a round of antibiotics. It has been suggested that probiotics be used to treat problems in the stomach and intestines. Make sure to keep them away from moisture and heat which can kill off some of the microbes. I recommend taking on an empty stomach, ideally right when you wake up. You should always store supplements in a cool, dark place but refrigeration is best. Most of the strains of probiotics are fragile and should be protected from heat- so refrigeration is ideal. Bacteria die out over time. Some supplements list the potency when they were manufactured (before they rode in a truck, sat on the shelf at the grocery store, or hung out in the kitchen cupboard for a few months). In this case, there could be dramatically fewer viable bacteria by the time you consume them than when they were first encapsulated, and good bacteria are no good to you dead.

Even if some of the bacteria in a probiotic managed to survive and propagate in the intestine, there would likely be far too few of them to dramatically alter the overall composition of one's internal ecosystem. Only one study—of 34 healthy volunteers—found a statistically significant change, and there was no indication that it provided a clinical benefit. “A probiotic is still just a drop in a bucket,” says Shira Doron, an infectious disease expert at Tufts Medical Center. “The gut always has orders of magnitude more microbes.”
